Insurance

 

I am contracted with several insurance providers. However, I am not in-network with all plans. Please contact your plan directly to confirm.

  • Cigna/Evernorth

  • United Healthcare / United Behavioral Health / Optum

I will also verify benefits/coverage for my enrolled clients and for any prospective clients. Please note that you are responsible for any fees that are not covered by your insurance plan. All fees are collected at the time of your appointment and can be made online through SimplePractice. 

Notice regarding no-shows/late cancellation (<24 hours):  Your insurance will not cover no-shows or late cancellations, and it is my policy to change the insurance provider’s contracted rate in these situations. You will be responsible for the full contracted rate (which depends on your particular insurance company) and will receive an invoice for this amount.

Self-Pay and Out-Of-Network Rates

For those paying out of pocket or using out-of-network insurance benefits, my billing rates are listed below.  This information only applies if I am not an in-network provider with your specific insurance plan.  Fees are due in full at the time of service.  Payment must be made by credit card (or HSA/FSA card) through the SimplePractice portal.  I will provide a Good Faith Estimate for anyone paying out of pocket.

General Fees

There are additional fees that may apply to some individuals. This information is provided in my practice policies document, which I provide to all new clients. Examples include report writing, records copying/printing, and overdue invoice fees.
